Off the top of my head, I remember the Week 17 game vs. the Bengals in 2004 that Koy started. Also, the year he broke his ankle, he was not on IR and came back for the playoffs.

I just looked into it, and McNabb wasn't on IR for the first game he missed in 2005 either.
