The Secretary of Defense has to concur the launch. But he can't veto it. The President has the authority to fire the SOD and put someone in place who will concur the launch. There's zero chance Mattis is approving a launch unless shtein is completely FUBAR. I would say an order to murder several million people might be considered unlawful, but then again, they select people who have no problem turning those keys, so I'd say if the Order is given, they'll launch without question.

if we were attacking first he would have to get congressional approval....if its a response he can give a direct order himself

the people punching in the code and turning the key are supposed to follow orders no matter what and would no doubt do it if an order reaches their station

if there was going to be something to stop him it would be the pentagon or the generals...they would have to basically say no and put him in a straight jacket which would basically be a military coup at that point

in other words we aren't launching any nukes unless attacked first which would never happen because leaders love their power more than dying
